About the position

Home and ALF visits | Palm Coast, FL Pay: \$65 per treatment (4-unit Hospice visit) and \$50 per (5 unit Part B treatment) Join Sobe Innovative Rehab, an outpatient therapy-at-home provider serving Assisted Living Facilities and home visits. We're looking for a compassionate, licensed Physical Therapist Assistant to provide high-quality geriatric therapy that helps residents maintain independence and age in place.

Responsibilities

• Provide skilled physical therapy treatments under the plan of care
• Help patients improve mobility, function, and independence
• Communicate effectively with physicians, families, and the care team
• Complete accurate, timely documentation

Requirements

• Associate degree from an accredited Physical Therapist Assistant program
• Active Florida PTA license

Nice-to-haves

• Two years of clinical experience preferred
